Results of a multi-faceted educational intervention to prevent peripheral venous catheter-associated bloodstream infections

末梢静脈カテーテル関連血流感染症は、入院の長期化、合併症および医療費増加をもたらす。スペインの大学病院 1 施設において、感染予防策が末梢静脈カテーテル関連血流感染症の発生率に及ぼす影響を評価した。2015 年に積極的サーベイランスプログラムを開始し、これにより末梢静脈カテーテル関連血流感染症の発生率比が高い（1,000 患者日あたり 0.48）ことが明らかになった。看護師、医療スタッフおよび患者を対象としたバンドルを実施し、カテーテル感染症チームを立ち上げた。この介入により、末梢静脈カテーテル関連血流感染症の発生率低下が達成され、2016 年の 0.34 から、2017 年には 0.29、2018 年には 0.17 となった。発生率の低下はグラム陰性菌による末梢静脈カテーテル関連血流感染症で最も大きかった（2015 年に 67.6％、2018 年に 35.3％）。

監訳者コメント：
末梢静脈カテーテル関連血流感染症（PVC-BSI）は、中心静脈カテーテル関連血流感染症（CVC-BSI）ほど、これまであまり注目されていなかった。本論文は PVC-BSI に注目し、サーベイランスを開始したところ、発生率が高いことが判明し、カテーテル感染症対策チームの組織と複合的対策バンドルを作成し、複合的な感染対策を実施した。バンドルは、①挿入手技前の手指衛生の遵守、②皮膚消毒をアルコール含有 2％クロルヘキシジングルコン酸塩に変更、③適切なカテーテルの選択、④消毒剤でごしごし擦ることによるポートへのアクセスと滅菌デバイスのみをポートにアクセスすること、⑤毎シフト毎に挿入部の観察を透明ドレッシング剤上から実施、⑥不要なカテは抜去し、毎日カテーテル治療の必要性を評価、であった。また 6 日以上の挿入が必要な場合は、PICC またはミッドラインカテーテルを選択した。、対策チームは、看護師 2 名、細菌検査技師 1 名、集中治療医 1名、内科医 2 名で構成され、看護師や看護助手のバンドル教育を実施し、その結果効果的に発生率を低下させることができた。
