背景
手指衛生は、微生物伝播の予防において重要な役割を担っている。オゾン(O3)は反応性の高い気体であり、細菌、ウイルス、および原生動物に対して広域の抗微生物活性を有する。オゾンは小型発生器により局所的に簡単に生成でき、水道水に溶解させると、周囲の空気中で通常の酸素(O2)に速やかに移行する。
目的
オゾンを溶解させた水道水と擦式アルコール製剤について、細菌で汚染させた手指の汚染除去能を比較すること。
方法
看護学生30 名を対象にクロスオーバー研究を実施した。参加者の手指を大腸菌(Escherichia coli : ATCC 25922)で人工的に汚染させ、次いでオゾンを溶解させた水道水(0.8または4 ppm)または標準的な擦式アルコール製剤(Antibac、アルコール85%)3 mL で消毒させた。手指に一時的に付着した微生物の培養を行い、コロニー形成単位(cfu)/mL を計数した。試験手順は、European Standard EN 1500:2013 を改変して用いた。
結果
汚染させたすべての手指で、消毒前の cfu は 30,000/mL を超えていた。細菌数(cfu/mL)の平均(SD)は、両手の手指を合わせて、オゾン水を用いた後は 1,017(1,391)、擦式アルコール製剤を用いた後は 2,337(4,664)であった。中央値(範囲)は、それぞれ 500(0 ~ 6,700)および 250(0 ~ 16,000)であった(差は非有意)。擦式アルコール製剤により参加者の 20%が有害な皮膚反応(灼熱感/乾燥)を報告したのに対し、オゾン水では有害な感覚の報告はなかった。
結論
オゾンを溶解させた水道水は、E. coli に対する有効な消毒剤であり、従来の擦式アルコール製剤の代替として、医療施設および公共区域の両方で用いることができると考えられる。オゾン水は、皮膚の問題を有する人にとっては特に価値あるものとなる可能性がある。

監訳者コメント:
オゾン水による手指や物品の消毒は、現在医療現場において第 1 選択の方策ではない。日本では 2000 年前後にオゾン水による消毒効果について報告が多数あり、現在でも実用的に使用している施設もそれなりにあるようである。今後、本研究のような英文のエビデンスが蓄積すれば、その使用頻度も増加するのかもしれない。一方でオゾン発生器による健康被害なども報告されており、注意が必要である。
