背景
医療関連感染(HAI)は患者ケアにおいて最もよく見られる有害事象の 1 つであり、かなりの罹患率および死亡率の原因となっている。
目的
ウクライナ、キエフの急性期病院における現在の HAI 有病率および抗菌薬耐性率に関する初の推定値を得ること。
方法
2014 年 1 月から 2016 年 12 月に、キエフの急性期病院 5 施設において前向きサーベイランスを実施した。HAI の定義は、米国疾病対策センターの全米医療安全ネットワークに従った。
結果
53,884 例の患者のうち、3,753 例(7%)の HAI が認められた。最も報告頻度の高い HAI は気道感染症(肺炎 19.4%、下気道感染症 4.1%)、手術部位感染(19.6%)、尿路感染(17.5%)、血流感染(10.6%)であった。入院中の死亡は HAI 症例のうち 7.2%で報告された。HAI から最も高い頻度で分離された微生物は、大腸菌(Escherichia coli)(15.9%)、黄色ブドウ球菌(Staphylococcus aureus)(14.8%)、腸球菌属(Enterococcus spp.)(10.2%)、緑膿菌(Pseudomonas aeruginosa)(8.9%)、クレブシエラ属菌(Klebsiella spp.)(8.9%)であった。黄色ブドウ球菌の 28.2%でメチシリン耐性が報告され、腸球菌の 14.2%はバンコマイシンに耐性を示した。全体としては、すべての腸内細菌科細菌(Enterobacteriaceae)の 35.1%は第 3 世代セファロスポリンに耐性を示し、最も高い耐性率は肺炎桿菌(K. pneumoniae)(53.8%)および大腸菌(32.1%)で見られた。
結論
病院における感染管理の優先事項には、手術部位感染、肺炎、血流感染および尿路感染の予防が含まれるべきである。これらの結果は、急性期病院における感染予防管理に関する必須要件を明らかにするのに役立つ可能性がある。

キエフといえばウクライナの首都であり、特にバレエが有名な美しき古都です。そんなところにも耐性菌の蔓延(腸球菌の 14.2%が VRE で、肺炎桿菌の 53.8%が第 3 世代セファロスポリン耐性!)が押し寄せています。残念なことです。
