Are systematic drain tip or drainage fluid cultures predictive of surgical site infections ?

手術部位感染症（SSI）の早期発見を目的としたドレーン先端または排液の計画的培養については議論の余地がある。そこで、清潔手術または準清潔手術において、ドレーン先端または排液の計画的培養の結果と SSI 発生との関連を検討することを目的とした。2017 年 3 月 31 日以前に発表された観察研究を、PubMed および Cat.inist のデータベースで検索した。清潔手術または準清潔手術後のドレーン先端または排液の計画的培養の結果と SSI について報告した研究を対象として、メタアナリシスを実施した。ドレーン先端培養施行患者 4,390 例および排液培養施行患者 1,288 例を含む 17 件の研究を選択した。統合陰性適中率は高かった（ドレーン先端培養では 99％、95％信頼区間［CI］98 ～ 100、排液培養では 98％、95％CI 94 ～ 100）。陽性適中率は低かった（ドレーン先端培養では 11％、95％CI 2 ～ 24、排液培養では 12％、95％CI 3 ～ 24）。感度は低かった（ドレーン先端培養では 41％、95％CI 12 ～ 73、排液培養では 37％、95％CI 16 ～ 60）。特異度はドレーン先端培養では高く（93％、95％CI 88 ～ 96）、排液培養では中程度（77％、95％CI 54 ～ 94）であった。ドレーン先端または排液の計画的培養は、SSI 予測の陽性的中率が低いため、重要ではなさそうである。

監訳者コメント：
ドレーン先端または排液の計画的培養、つまりドレーンを挿入した患者に対して監視培養検査を行うことの診断的メリットについて文献調査した報告である。本報告の意味するところの解釈が難解であるが、清潔手術における監視培養の是非についてはよく検討する必要がある。
