目的
結腸直腸の待機的腹腔鏡下手術を受ける患者において、従来の創被覆材とビタミン E・シリコン創被覆材が切開手術部位感染症(SSI)に及ぼす効果を比較すること。
患者および方法
前向き無作為化試験を実施した。患者をビタミン E・シリコン創被覆材群と従来の創被覆材群の 2 群に無作為に割り付けた。切開部 SSI、術後疼痛、急性期反応物質について調べた。
結果
患者計 120 例を本研究に組み入れた(各群 60 例)。切開部 SSI 発生率は、ビタミン E・シリコン創被覆材群では 3.4%、従来の創被覆材群では 17.2%であった(P = 0.013)。ビタミン E・シリコン創被覆材群における感染創の培養ではバクテロイデス フラジリス(Bacteroides fragilis)のみの増殖がみられたのに対し、従来の創被覆材群の感染創の培養では複数菌の増殖がみられた。術後 48 時間の術後疼痛の平均は、ビタミン E・シリコン創被覆材群では 27.1 mm(標準偏差[SD]10.7 mm)、従来の創被覆材群では 41.6 mm(SD 16.9 mm)であった(P < 0.001)。術後合併症を呈した患者を除外しても、ビタミン E・シリコン創被覆材群の方が白血球数(WBC)およびC反応性タンパク質(CRP)は低値であった。
結論
結腸直腸の待機的腹腔鏡下手術後の Pfannestiel 切開創を被覆するのにビタミンE・シリコン創被覆材の使用は、切開部 SSI 発生率の低下、術後疼痛の軽減、CRP 値およびWBC 数の低下をもたらした。
