Can guidelines for the control of multi-drug-resistant Gram-negative organisms be put into practice? A national survey of guideline compliance and comparison of available guidelines

多剤耐性グラム陰性菌は、新たな世界的脅威となっており、これはアイルランドや各国における感染症発生率の上昇に反映されている。この脅威への対処は、感染予防・制御（IPC）ガイドラインの策定である。国内ガイドラインの遵守について評価するために、アイルランドの IPC チームへの調査を実施した。これらの調査結果を状況の中で捉えるために、アイルランドの Health Protection Surveillance Centre の IPC ガイドラインを、Healthcare Infection Society、欧州臨床微生物学・感染症学会、米国疾病対策センターのガイドラインと比較した。さまざまな種類の病院の 33％から回答を得た。その結果から、アイルランド全域におけるガイドライン実践のばらつきとともに、国際的にもガイドライン間の相違が浮き彫りにされた。多剤耐性グラム陰性菌スクリーニングのガイドラインの大部分を遵守しているという回答は 90％未満であった。病院間で隔離室の利用に差があり、利用可能な個室は平均 29％（2.6 ～ 100％）であり、多剤耐性グラム陰性菌を有する患者によっては、隔離されないこともある。個人防御用装備に関するガイドラインの適用に大きなばらつきがみられた。この調査によって、Health Protection Surveillance Centre ガイドラインの適用について実態を知ることができる。多剤耐性グラム陰性菌の IPC のための 5 種類のガイドラインの比較によって、調査結果を状況の中で捉える。 IPC の主な原則は、ガイドライン全体を通して標準的であるが、多剤耐性グラム陰性菌伝播の低減においてどのような実践が有効かつ費用効果的であることを示す研究に、意義があると思われる。

ガイドラインは法律ではなく、記載内容の遵守には当然限界がある。法的拘束力を持たないガイドラインを遵守させるには、何らかのインセンティブが必要となる。病床稼働の観点から個室運用がままならない医療機関にとって理想論ばかりかき立てるガイドラインを遵守するゆとりはなく、国策としての改善策が求められる。
