多剤耐性カンジダ・オーリス(Candida auris)によるアウトブレイクが、現代医学における大きな脅威として登場してきた。皮膚保菌および環境汚染がアウトブレイクの予測因子であることが明らかにされているため、著者らは C. auris 臨床分離株を用いて紫外線 C 波(ピーク波長254 ± 2 nm の水銀蒸気ランプを使用)、およびオクテニジン二塩酸塩の抗真菌活性について評価を行った。オクテニジン二塩酸塩は、臨床現場で現在用いられている濃度(0.05 ~ 0.1%)よりはるかに低い濃度(0.00005 ~ 0.0004%)で効果を有することが認められた。走査電子顕微鏡画像から、0.0005%オクテニジン二塩酸塩への曝露により 6 時間以内で C. auris 分離株が破壊されたことが示された。紫外線 C 波により、15 分間の曝露ですべての C. auris 分離株を殺滅できた。

監訳者コメント
オクテニジン二塩酸塩は、細胞膜・細胞壁の変性を生じ、グラム陽性菌、陰性菌、酵母に対し広い活性を有するとされる。本誌上においても MRSA の除菌結果が報告されたことがある(Journal of Hospital Infection 2010; 74, 199-203)。さらにクロルヘキシジンと同様の残存効果に加え、抗バイオフィルム効果を有し、既に創洗浄剤などで商品化されている。デバイスや器材表面上の状態での検討が今後求められるが、1 つの方法として、その効果と有用性、現実的な応用に関し、注目される薬剤と思われる。
