Impact of penicillin allergy records on carbapenem prescribing: an observational retrospective cohort study

ペニシリンアレルギーがあるという記録は、次に選択される抗菌薬に影響を及ぼす。本研究では、ペニシリンアレルギーの記録がメロペネムの処方に及ぼす影響について調査した。メロペネムの処方率を、ペニシリンアレルギーの記録がある患者とない患者とで比較し、考え得る交絡因子についても情報を収集した（すなわち、年齢、性別、および併存疾患）。メロペネムの処方を受けていたのは、ペニシリンアレルギーのない患者 21,272 例中 225 例（1.06％）に対し、ペニシリンアレルギーの患者では 3,443 例中 240 例（6.97％）であった。メロペネムの処方は、患者のペニシリンアレルギーの記録と相関する。多くのペニシリンアレルギーの記録が不正確であることを考えると、誤ったペニシリンアレルギーの記録という問題に取り組むことで、メロペネムの処方を減らせる可能性がある。

監訳者コメント：
ペニシリンアレルギーがあるという申告と記載には、しばしば誤りが含まれている。このような不正確な情報に基づいてペニシリン系を避け、広域抗菌薬を選択してしまう症例も実際多い。中にはカルバペネム系が選択されてしまうケースさえあり、その使用増加によるデメリットも大きい。本検討にはペニシリンアレルギーの内容と強さ、回避した理由の詳細は含まれていないが、ペニシリンアレルギーそのものの扱いを不適切な抗菌薬の使用を避けるという意味からも見直すという認識が、今後広まってほしいものである。
