Intestinal persistence of a plasmid harbouring the OXA-48 carbapenemase gene after hospital discharge

退院後における OXA-48産生肺炎桿菌（Klebsiella pneumoniae）の腸管保菌について検討するため、保菌歴のある被験者 22 例から便サンプルを採取した。退院後の期間は 33 日から 611 日で、再入院はなかった。8 例（36％）が blaOXA-48 遺伝子の保有者と同定された。これら全例において、病院で獲得された OXA-48 産生肺炎桿菌株は消失しており、blaOXA-48 遺伝子を有していたのは他の肺炎桿菌、クレブシエラ・オキシトカ（Klebsiella oxytoca）および／または大腸菌（Escherichia coli）株であった。本研究の所見から、抗菌薬療法を受けていない人のかなりの割合で、腸管内において OXA-48 カルバペネマーゼ遺伝子を有するプラスミドの存在が数か月間にわたり持続することが示された。

監訳者コメント：
薬剤耐性腸内細菌科細菌の保菌期間に関しては様々な研究がある。本研究の興味深いところは、耐性菌は菌株としては消失していたものの、耐性遺伝子としては他の菌株に受け継がれて保存されていたという点である。ただしこれらの結果が臨床的にどのような意味があるかは今後の検討が必要である。
