湿潤表面のバイオフィルムは、カルバペネマーゼ産生腸内細菌科細菌（CPE）を含む多剤耐性グラム陰性菌のリザーバの 1 つである可能性がある。環境的な感染源の認識は、2 次的な患者への伝播を減少させる上で重要である。熱傷病棟のシャワールームの床排水管由来の環境サンプルにおいて、清掃後に採取した場合は清掃前に採取した場合と比較して、blaIMP-4^＋ CPE の検出増加がみられたことを報告する。清掃中のバイオフィルムの破壊が多剤耐性菌の検出増加の原因である可能性を提案する。今回の結果により、湿潤環境は認知度は低いが、CPE 伝播の潜在的原因の 1 つという役割を持つことが浮き彫りになった。
清掃前のサンプルに焦点を合わせた環境スクリーニングのみでは、環境汚染を過小評価してしまう可能性が高い。

監訳者コメント：
なんと、清掃「後」の方が、清掃「前」よりも、検出される耐性菌の菌量が「増加」するという論文である。清掃によってバイオフィルムを破壊した方が、その中の菌の検出が増加するというのである。この発想はなかったな…「清掃前のサンプルだけで評価してはいけない」ということで、「清掃したから大丈夫だろう」という発想は、特にバイオフィルムを形成する細菌の場合は当てはまらないということだろうか。今後の同様の研究に注目である。
