Vagino-rectal colonization and maternaleneonatal transmission of Enterobacteriaceae producing extended-spectrum β-lactamases or carbapenemases: a cross-sectional study

本研究の目的は、横断研究デザインを用いて、母親 815 例および新生児 800 例を対象に、基質特異性拡張型βラクタマーゼ産生腸内細菌科細菌（ESBL-E）およびカルバペネマーゼ産生腸内細菌科細菌（CPE）について存在率およびリスク因子を明らかにすることであった。ESBL-E を保菌していたのは女性 59 例および新生児 13 例であった（それぞれ存在率 6.7％、95％信頼区間［CI］5.2 ～ 8.7 および 1.6％、95％CI 0.7 ～ 2.5）。CPE は検出されなかった。最も高頻度に認められた ESBL-E は CTX-M-14 および SHV-12 であった。垂直伝播は、保菌していた母親の 14％ で発生していた。保菌のリスク因子は、母親については過去の妊娠における合併症、1 回以上の尿路感染症の既往、非白人、および主要な食事を外食している頻度が高いことであり、新生児については母親の保菌および経腟分娩であった。
