医療関連グラム陰性菌血流感染症の季節、気象、予測因子:症例のみの研究
Season, weather and predictors of healthcare associated Gram-negative bloodstream infections: a case-only study

背景
近年の研究では、医療関連感染症(HCAI)の季節性について報告されている。この現象と HCAI の他のリスク因子との関連は明らかではない。
目的
グラム陰性桿菌による医療関連血流感染症(BSI)の季節、気象、および通常の予測因子の相互作用を分析すること。
方法
ブラジルの教育病院において、症例のみの研究を実施した。2012 年 7 月から 2016 年 6 月にグラム陰性桿菌による BSI と診断された患者 446 例を研究に登録した。人口統計学的データ、併存疾患、侵襲的処置、抗菌薬使用についてカルテをレビューした。グラム陰性桿菌による BSI が発生した季節、診断日の気象パラメータを記録した。季節ごと(基準カテゴリー:冬)のグラム陰性桿菌による BSI および種々のグラム陰性桿菌(基準カテゴリー:大腸菌[Escherichia coli])による BSI の発生と関連する因子を分析した。単変量および多変量多項ロジスティック回帰モデルを用いて分析した。
結果
夏に診断されたグラム陰性桿菌による BSI は、クレブシエラ(Klebsiella)属菌(オッズ比[OR]5.33、95%信頼区間[CI]2.04 ~ 13.96)やアシネトバクター・バウマニー(Acinetobacter baumannii)(OR 2.69、95%CI 1.04 ~ 6.96)に起因する割合が高く、クレブシエラ属菌と春の関連がみられた(OR 2.86、95%CI 1.14 ~ 7.18)。診断日の平均気温は、クレブシエラ属菌(OR 1.19、95%CI 1.07 ~ 1.33 )、A.baumannii(OR 1.20、95%CI 1.07 ~ 1.34)と関連した。
結論
通常のリスク因子を補正したモデルにおいても、暖候期および毎日の気温が、グラム陰性桿菌による BSI の病因に影響を及ぼした。これらの結果に関する説明の 1 つとして、医療関連病原体の季節性は微生物に本来備わっているもので、併存疾患、処置、または抗菌薬使用と関連しないということが考えられる。
