滅菌可能な医療器具の再処理における 6 時間推奨を検証する

背景
現在、滅菌可能な医療器具の再処理は、器具の品質が低下しないことが保証される手術終了後 6 時間以内に開始するよう推奨されている。文献調査では、医療従事者が 6 時間という標準的な滅菌プロトコールを遵守しない場合に起こりうる結果のエビデンスが不十分であった。
目的
滅菌可能な医療器具の再処理における 6 時間推奨について、再処理前の待機時間に比例して残留蛋白が増加しているか否か、同様に、待機時間に比例して外科用剪刀の腐食が増加しているか否かを測定することにより評価する。
方法
ヒト血液で汚染された外科用器具について、さまざまな待機時間後および洗浄前に、オルトフタルジアルデヒド法により残留蛋白を測定した。ヒト血液で汚染された外科用剪刀について、さまざまな待機時間後および再処理後に、光学顕微鏡・走査型電子顕微鏡により腐食を測定した。
結果
蛋白の残留は 14.0 μg ~ 51.9 μg の範囲内であり、器具表面あたりの許容閾値 100 μg を下回っていた。外科用剪刀 30 本のうち 22 本で、表面の 0.05%に相当する腐食が確認され、30 本のうち 4 本で孔食がみられた。
結論
残留蛋白と待機時間との関連も、腐食の発生と滞留時間との関連も確認されなかった。よって、本研究では、再処理前の最大待機時間 6 時間という推奨を支持する妥当性を検証できた。この結果は、デンマークおよび国際的にも、外科用器具の再処理の取り組みに影響を及ぼす可能性がある。
