Targeted management of influenza A/B outbreaks incorporating the cobas^® Influenza A/B & RSV into the virology laboratory

2017 年から 2018 年のインフルエンザシーズンの間に、著者のウイルス検査室は、インフルエンザアウトブレイクの管理のために、2 つのシナリオ、すなわち初期のアウトブレイク調査と、アウトブレイクにおいて長期に及ぶ対策を回避する決定において、cobas^® Influenza A/B & RSV（Roche Molecular Diagnostics, Pleasanton, CA, USA）を利用した。27 件の調査を実施し、そのうち 11 件はインフルエンザ A ／ B のアウトブレイク宣言であった。調査の 30％は、検査室が独自策定した、まとめて 1 日 1 回行う標準的な呼吸器検体のポリメラーゼ連鎖反応（PCR）の機会を逸した可能性があり、アウトブレイクの確定が翌日まで遅延した。インフルエンザ A ／ B 検査の所要時間短縮の平均は 10.2 時間であった。特定のアウトブレイクシナリオにおける迅速分子 PCR によって、インフルエンザアウトブレイクの時宜を得た管理が改善された。

監訳者コメント：
検査による診断も大切だが、シーズン中にインフルエンザ様症状を呈したら、まず対応し、その後インフルエンザが確実に否定されたら解除することが、拡大防止において重要であろう。そのためにも、検査部門と感染管理部門の密な連携が求められる。
