背景
結核菌(Mycobacterium tuberculosis)は、世界的に重大な保健衛生上の負担となっている。この感染症は、個々の症例および市中のアウトブレイクとして発生する他、まれに院内アウトブレイクを生じる。英国のような有病率が低い国であっても、結核は医療従事者にとってリスクをもたらす。
目的
Queen Elizabeth Hospital Birmingham において、肺結核を有する患者 1 例が入院した 12 か月後に発生したアウトブレイクを報告すること。
方法
疫学調査とアウトブレイクの調査を行うとともに、アウトブレイクのさらなる拡大防止を目的とした、アウトブレイクの同定と制御策の決定における全ゲノムシークエンシングの役割を明らかにし、報告した。
結果
患者 1 例で開放性結核の診断が下された後、医療従事者 1 名が活動性結核を有しており、伝播が確認され、また初発患者に接触した医療従事者 7 名において潜在性結核感染症が認められた。注目すべき点として、潜在性結核感染症の患者全員が、結核の他のリスク因子を有していた。全ゲノムシークエンシングのルーチンの使用により、初発患者と活動性結核を有する医療従事者とのアウトブレイク上の関連が同定され、著者らの調査に情報を提供した。
結論
アウトブレイク当時の国内ガイドラインに従って呼吸器防護具を用いずに実施されたエアロゾル発生処置の際に、曝露が生じた可能性が極めて高い。アウトブレイク後、強化した感染制御策が実施されるようになった。

監訳者コメント:
開放性結核患者との接触から 12 か月で医療従事者に活動性結核が発症した事例である。診断・検査・感染対策の開始の遅れがあり、結核罹患が判明した医療従事者はすべて、十分な感染対策を行わない状態でエアロゾルを発生させる処置に携わっていた。中には 1 日だけではあるが、治療開始から 14 日経過したために感染対策を解除した日があり(後になってこの日の検体も塗抹陽性が判明)、この日の接触で感染してしまった例があった。実際の結核の院内感染対策を考えるうえで、教訓となる事例である。
