A survey on current knowledge, practice and beliefs related to preoperative antimicrobial decolonization regimens for prevention of surgical site infections among Austrian surgeons

背景：
手術部位感染症（SSI）のリスクを低減させるために、術前除菌などさまざまな方法が検討されている。外科部門における術前除菌の実施の詳細について、オーストリアでは研究されていない。
目的：
国内の病院における患者の術前除菌の現状を分析することと、異なる外科領域の外科医を対象に、現時点でのこの方法に関する認識を評価すること。
方法：
複数の外科部門を有するオーストリアの全病院に 12 項目の構造化質問票を配布した。
結果：
回答した外科医の 3 分の 2（158 名中 103 名、65％）が、所属する外科部門でいずれかの種類の術前除菌を実施していると記載した。プロトコールの多様性がみられ、既知の黄色ブドウ球菌（Staphylococcus aureus）保菌者のみの除菌から、患者のサブグループの除菌、あるいは待機的手術前のすべての患者における普遍的除菌などさまざまであった。もっとも使用頻度の高い抗菌性化合物はオクテニジン（60.2％）で、ムピロシン（38.8％）、トリクロサン（14.6％）、ポリヘキサニド（12.6％）、クロルヘキシジン（11.7％）、塩化ジデシルジモニウム（7.8％）と続いた。
結論：
オーストリアの病院では術前除菌がルーチンベースで実施されているようである。とはいえ、この方法は、さまざまなプロトコール・抗菌性化合物・職員によって実施されている。今回の調査から、術前除菌についてより情報を得ている外科医は、予防法の有用性をより確信している外科医でもあるということが確認されたので、今後の活動は、この領域でのより多くの比較研究の実施に焦点を当てるだけでなく、的を絞った教育も対象とすべきである。

監訳者コメント：
オーストリアの病院では、術前除菌がルーチンベースで実施されているという事実だけでなく、除菌に使用している薬剤の耐性菌についても調べていく必要があるだろう。オクテニジンは、日本では除菌目的の使用はなされていない。
