背景
集中治療室の意図しない移設が、OprD 介在性のイミペネム耐性を示す緑膿菌(Pseudomonas aeruginosa)(PA-OprD)による保菌/感染症のリスクに及ぼした影響を評価すること。
目的
主要目的は、緑膿菌分離株における PA-OprD の割合を、集中治療室の意図しない移設の前後で比較することであった。副次的目的は、PA-OprD の保菌/感染症を有する患者に対する汚染経路としての水道水の役割を評価することであった。
方法
2013 年 10 月から 2015 年 10 月にかけて単一施設観察前後比較研究を実施した。2014 年 10 月末に集中治療室が移設された。集中治療室に 48 時間以上入院していた患者から分離されたすべての緑膿菌株を対象とした。集中治療室において水道水検体を 3 か月ごとに採取した。患者および水道水から分離された PA-OprD 株について、パルスフィールド・ゲル電気泳動により遺伝子型決定を行った。
結果
合計で緑膿菌臨床分離株 139 株および水道水検体 19 個について分析を行った。PA-OprD 株の割合は、31% から集中治療室移設後には 7.7%に低下した(P = 0.004)。すべての PA-OprD 臨床分離株についてそれぞれ独自の遺伝子型が認められた。驚くべきことに、水道水には移設前後の期間において単一の PA-OprD 株の定着が認められたが、この単一クローンは臨床検体からは分離されたことがなかった。
結論
集中治療室の移設は、イミペネム耐性緑膿菌株の顕著な減少と関連した。患者から分離された PA-OprD 株が多クローン性であったこと、また水道水から患者への汚染伝播が認められなかったことから、緑膿菌による内因性の保菌/感染症に及ぼす環境の影響は複雑であることが示される。

監訳者コメント
集中治療室移設の前後での、緑膿菌の臨床分離株の比較および、水道水中の緑膿菌の検出を比較を行った論文である。移設後も、水道水から単一株の緑膿菌が検出されたことに疑問を感じた。
