背景
黄色ブドウ球菌(Staphylococcus aureus)の鼻咽頭保菌率は集団内において 20 ~ 30%にも達することがあり、侵襲性感染症の原因となり得る。
目的
様々な年齢群における保菌率を調べ、黄色ブドウ球菌株に特異的な病原性パターンを分析すること。
方法
保菌率の分析を行うため、新生児、5 ~ 60 歳の健康ボランティア、および 80 歳超のナーシングホーム居住者を含む年齢群を対象として鼻咽頭スワブを調べた。黄色ブドウ球菌が回収された場合、遺伝子解析および病原性に関する表現型検査を細胞アッセイにより実施した。
結果
ボランティア 924 例において全保菌率は約 30%で、5 ~ 10 歳の被験者で最も高かった(49%)。新生児および 80 歳超の被験者では、クローンクラスターの分布に差が認められた。全体的にすべての年齢群の分離株で、感染症の発症を引き起こし得る病原性の特徴が示された。特に、新生児由来の分離株では、検査の対象とした他の分離株より高い細胞毒性作用をもたらした毒性遺伝子の保有率が高いことが示された。
結論
保菌由来の分離株にはすべての年齢群で病原性の特徴が認められ、これは侵襲性感染症の発症につながる可能性がある。したがって、感染症のリスクに応じて選択した患者に対して、除菌の方策を検討できるであろう。

監訳者コメント
ドイツの健常人における黄色ブドウ球菌の保菌率と遺伝子解析を実施した論文である。5 ~ 10歳で保菌率が高いのは集団生活の影響があると思われるが、約 50%という結果には驚いた。
