Invasive device-associated infections caused by Pseudomonas aeruginosa in critically ill patients: evolution over 10 years

2007 年から 2016 年の 10 年にわたり緑膿菌（Pseudomonas aeruginosa）が原因で引き起こされた侵襲的デバイス関連感染を、ENVIN-HELICS レジストリ（スペインの集中治療室 200 室）のデータに基づいて評価した。緑膿菌のわずかな減少が見られ大腸菌（Escherichia coli）が病因の首位であった直近の 2 年間を除き、緑膿菌は首位の病原体であった。研究期間を通して、緑膿菌に起因する感染症の割合は 12.0％から 14.6％の間のままであった。イミペネム、メロペネム、セフタジジム、セフェピム、タゾバクタム・ピペラシリンに対する耐性株は有意な増加が認められた。多剤耐性株、ならびに超多剤耐性株と汎多剤耐菌株の合計も増加した。抗緑膿菌抗菌薬に対する耐性は依然として懸案事項の 1 つである。

監訳者コメント：
スペインでは ICU で発生した感染症のサーベイランスシステムが稼働しており（ENVIN-HELICS レジストリ）、本検討ではこのデータベースの中から VAP、血管内カテーテル関連菌血症、カテーテル関連尿路感染症を侵襲的デバイス関連感染症として集計している。そして VAP の 19％、カテーテル関連菌血症の 6％、カテーテル関連尿路感染症の 12％が緑膿菌によるものであったが、驚いたことにカルバペネム系の約半数、セフタジジム、セフェピム、タゾバクタム・ピペラシリンの約 4 割が既に耐性になっていた。ICUにおける重症な医療関連感染の治療手段が失われる過程をみているようで、慄然とした思いにかられる。
