Effect of chlorhexidine baths on central-line-associated bloodstream infections in a neonatal intensive care unit in a developing country

新生児において、医療関連感染、特に中心ライン関連血流感染（central-line-associated bloodstream infections；CLABSI）は悩ましい問題である。本研究では準実験的研究を介して、発展途上国（コロンビア）のNICU 1 室におけるクロルヘキシジン浴の CLABSI 率への影響を示す。準実験的研究は 62 か月（2012 年 1 月から 2017 年2月）にわたり実施され、クロルヘキシジン浴のプロトコールが実行された 2014 年 7 月の前後で 2 期に分けられた。1,000 中心ライン日あたりの CLABSI 率は、プロトコールの実行後に 8.64 から 4.28 へ低下した。クロルヘキシジン浴の導入は新生児における CLABSI の件数を減少させるように思われる。

監訳者コメント：
本検討では 2％クロルヘキシジンに浸漬した 2 枚の清拭布を用い（1 枚は上半身用、もう1 枚は下半身用）、体重 1 ㎏以上は生後 2 日目から 1 日 1 回、1 ㎏未満は生後 7 日目から週 2 回、清拭を行った。清拭による物理的除去が有効であった面もあると思われ、S. epidermidis、Enterobacter、Serratia 菌血症では減少がみられた。しかしながらS. aureus、Klebsiella 菌血症は増加しており、総じて菌種をまたいだ減少を示した訳ではなかった。NICU 内での S. aureus、Klebsiella 発生について情報はないが、清拭でコントロールできる範囲は限定的で、NICU 内の種々の状況を勘案した、発生抑制につながる複数の方策を併用することではじめて CLABSI が抑制できることを改めて示唆しているように思われる。
未熟児・新生児におけるクロルヘキシジンの使用については、皮膚組織への影響や体内に吸収されての影響など今後評価を十分におこなっておく必要がある。安易に試みるべき、予防措置ではない。
