Sustained improvement in hand hygiene compliance using a multi-modal improvement programme at a Swiss multi-site regional hospital

背景
手指衛生遵守の持続的改善に関する施設レベルでのエビデンスは乏しい。
目的
スタッフの手指衛生遵守に対して病院全体で実施された改善プログラムの影響および持続可能性を評価する。
方法
スイスのヌーシャテル州にある複数施設を有する 450 床の教育病院内で、すべての臨床スタッフの手指衛生遵守の傾向を、訓練を受けた観察者の直接観察により測定して分析すること。
介入
世界保健機関（WHO）の戦略に基づく多面的改善プログラムを実施し、全体の遵守率が少なくとも 80％に達することを目標とした。戦略の内容には、擦式アルコール製剤の利用可能性の向上、医療従事者の教育、2 か月ごとの遵守率測定、病棟レベルの結果についての病院内でのオープンな通知、ポイント・オブ・ケアでのリマインダー、専用の院内ニュースレターによる通知、リーダーの動機づけが含まれた。実施期間の後に、定着化期間が設定された。
結果
2012 年 9 月から 2014 年 3 月までに、合計で 33,476 件の観察が行われた（監査あたり平均 3,000 件を超える機会）。全体的な遵守率は、ベースライン時の 61.4％から 18 か月間の改善プログラム後には83.6％に改善し（P ＜ 0.001）、さらに18 か 月後（すなわち、プログラム終了後18 か月後）に 85.3％と持続が認められた（P ＝ 0.08）。同じ傾向（介入期間中の有意かつ臨床的に意味のある改善、18 か月後に持続）がすべての職種で認められた。
結論
WHO の戦略に基づくこの改善プログラムにより、病院全体および各職種において手指衛生遵守率が有意に改善した。この結果は介入後 18 か月間にわたり持続した。
