Does screening neonates in the neonatal intensive care unit for Pseudomonas aeruginosa colonization help prevent infection?

背景
緑膿菌（Pseudomonas aeruginosa）はグラム陰性の環境微生物で、早産児を含めた免疫機能低下患者において重度の感染症を引き起こし得る。近年、新生児に対して緑膿菌保菌のスクリーニングを行う実践が一般的になりつつある。
目的
新生児に対する緑膿菌スクリーニングについて、（1）重度の緑膿菌感染症を発症するリスクの予測、および（2）感染制御実践の指針としての利用における価値を評価すること。
方法
2012 年 8 月から 2015 年 9 月までの間にNorth Bristol NHS Trust で新生児集中治療室（NICU）に入室した新生児に対して、入室時およびその後週 1 回のルーチンの緑膿菌保菌スクリーニングが実施された。緑膿菌感染症を発症した乳児に関するデータも収集した。NICU で採取された環境サンプルを緑膿菌の有無について検査した。新生児および環境から回収された緑膿菌のすべての分離株に対して variable number tandem repeat（VNTR）タイピングを実施した。
結果
スクリーニング結果陽性の新生児で、その後に緑膿菌感染症を発症した新生児はいなかった。環境または他の新生児からの交差感染を支持する VNTR 所見は得られなかった。
結論
新生児に対する緑膿菌スクリーニングにより、その後に緑膿菌感染症を発症する新生児は特定されなかった。2015 年 9 月にスクリーニングを中止後、緑膿菌感染症を有すると同定された新生児の人数は増加しなかった。

監訳者コメント：
日本でも NICU 滞在患児の監視培養を行っている施設は多いが、私の知るところではその目的は様々で、MRSA をターゲットとして監視培養を行っている施設もあれば、とりあえずどんな菌でも拾い上げるような監視培養をしているという施設もある。日本では緑膿菌をターゲットとして監視培養を行っている施設は少ない印象であるが、本論文によるとイギリスではそのような監視培養がそれなりに行われているということである。本研究はイギリスで過去に緑膿菌のアウトブレイクが発生した NICU で行われた緑膿菌検出目的の監視培養に関する研究であるが、監視培養の意義は乏しいという結論であった。
