Point-prevalence survey of carbapenemase-producing Enterobacteriaceae and vancomycin-resistant enterococci in adult inpatients in a university teaching hospital in the UK

カルバペネマーゼ産生腸内細菌科細菌（CPE）やバンコマイシン耐性腸球菌（VRE）による感染症は、罹患率および死亡率の増加と関連するが、入院患者における CPE と VRE の保菌率は不明である。成人入院患者の CPE と VRE の保菌率を明らかにするために点有病率調査を実施した。成人入院患者 960 例のうち 818 例（85.2％）に本試験への参加を依頼した。このうち 595 例（72.7％）から同意を取得し、試料を得た。検査した 540 サンプルのうち CPE 陽性のものはなかった。540 サンプル中 130 サンプル（24.1％）は VRE 陽性で、40 病棟中 34 病棟（85％）で感染症が認められた。有病率の低い施設では、CPE のユニバーサルスクリーニングの費用対効果が高くない可能性があり、むしろ高リスク患者を対象としたスクリーニングを継続すべきである。当研究環境ではユニバーサルスクリーニングや隔離は不可能なので、VRE スクリーニングのための最適な対策はまだ確立されていない。

有病率を踏まえたスクリーニングのあり方について検討した論文である。米国では、一部の大学病院で、菌保有者の接触予防策を中止し、（ユニバーサルに）標準予防策を行うという取り組みが検討されている。有病率と費用対効果を踏まえた感染症対策に関する検討は医療現場にとって大変有意義である。
