背景
カルバペネム耐性腸内細菌科細菌(CRE)保菌の自然持続期間、および持続的保菌と関連する因子に関する情報は限られている。
目的
肺炎桿菌(Klebsiella pneumoniae)カルバペネマーゼ(KPC)産生 CRE の持続的保菌と関連する臨床変数を評価すること。
方法
2015 年 6 月から 2016 年 12 月に入院し、直腸スワブまたは臨床培養により KPC 産生 CRE 保菌者と特定された患者のデータを後向きにレビューした。初回獲得後 3 か月間の追跡培養データを有する患者を対象とした。持続的保菌と関連する臨床変数を評価するために回帰モデルを用いた。
結果
適格患者 100 例のうち 50 例(50%)で 3 か月以内に自然除菌が認められた。3 か月後も培養陽性であった 50 例(50%)のうち 26 例は、6 か月後も KPC 産生 CRE を保菌していた。多変量解析では、再入院(補正オッズ比[aOR]9.96、95%信頼区間[CI]1.13 ~ 87.98、P = 0.039)、入院期間(aOR 1.03、95% CI 1.01 ~ 1.05、P = 0.003)、臨床培養陽性(aOR 6.26、95% CI 1.28 ~ 30.54、P = 0.023)、カルバペネム使用(aOR 9.15、95% CI 1.85 ~ 45.27、P = 0.007)が、6 か月後の持続的保菌の予測因子であった。
結論
結果から、臨床検体に KPC 産生 CRE が検出されたカルバペネム使用中の患者、特に入院回数が多く、入院期間が長い患者は、初回獲得から 6 か月後も保菌状態が持続する可能性が高いことが示唆される。今回の情報は、CRE 保菌状態の適切な予測により先制攻撃的隔離の対策を調整するうえで、またリスク因子の層別化により積極的サーベイランスを確実にするうえで有用である。

監訳者コメント
直腸スワブまたは臨床培養により KPC 産生 CRE 保菌者と特定された患者のデータから持続的に保菌する要因について調べた論文である。同様の研究はこれまでにも報告されているが、臨床検体から検出された患者で長期間保菌状態が続いたことは、対策を考える上で、重要な因子であると思われた。
