小児および思春期児童でのクロストリジウム・ディフィシル(Clostridium difficile)関連下痢症の予防における経口プロバイオティクスの費用効果分析★

背景
入院している小児および思春期児童におけるクロストリジウム・ディフィシル(Clostridium difficile)関連下痢症(CDAD)の発生率は年々増加している。小児の CDAD は、医療システムに大きな経済的負担をかけている。プロバイオティクスは生菌で、宿主の微生物バランスを改善し、腸内細菌叢の乱れを是正し、病原性細菌の保菌リスクを低減すると考えられる。
目的
費用効果分析を行い、抗菌薬投与を受けている小児および思春期児童における CDAD 予防を目的としたプロバイオティクスの経済性を評価した。
方法
臨床的有効性、効用および費用のデータを組み合わせた決定木モデルを使用した。感度分析を行ってモデルの結果の頑健性を判定した。
結果
「経口プロバイオティック」戦略および「プロバイオティクスなし」戦略は、患者にそれぞれ 0.05876 質調整生存年(QALY)および 0.056 QALY がもたらされ、それぞれにかかった費用は16,668.70ドルおよび20,355.28ドルであった。経口プロバイオティクス戦略はより高い QALY とより低いコストを示し、費用を節減する戦略である。この結果は、感度分析により頑健であることが示された。
結論
医療システムの観点からは、入院して抗菌薬療法を受けている小児および思春期児童における CDAD 予防戦略としての経口プロバイオティクスは、CDAD のリスクを低減するとともに、費用を節減する戦略である。

これまでプロバイオティクスの CDAD 発症の予防効果については成人でのものがあるが、小児でのデータは極めて少ない。プロバイオティクスの投与により小児における CDAD 発症を予防することができるかという命題に対して、過去の論文のメタアナリシスにより有効性を検証しているが、使用されたプロバイオティクスは 4 つの研究において異なっており、個々での効果を検証することはできていない。しかしながら総じてプロバイオティクスは小児における CDI のコストを下げることができており、今後個々のプロバイオティクスでの予防効果の研究が待たれる。
