Molecular epidemiology and antimicrobial resistance of Clostridium difficile in a national geriatric hospital in Costa Rica

背景
クロストリジウム・ディフィシル（Clostridium difficile）感染症（CDI）の疫学は世界中で急速に変化している。しかし、高齢者専門病院で流行している C. difficile の遺伝子型および抗菌薬への耐性に関する知識は不十分または存在しない。
方法
コスタリカの国立高齢者病院 1 施設において、CDI の分子疫学に関する前向き研究を 11 か月間実施した。
結果
対象患者は様々な併存疾患を有していたが、いずれも CDI と関連していなかった。これらの症例で得られた 32 株を解析すると、3 つの新しいリボタイプを含む多クローン性が判明し、かつ抗菌薬に対して高レベルの耐性を有していることが明らかになった。このような菌株の多様性にもかかわらず、最も高頻度のタイプは NAP6／RT002 および NAP2／RT001 であった。NAP9／RT017 は市中獲得に関連していた。なお患者の下痢発症前に 19 種類の抗菌薬が使用されていた。特定の遺伝子型が感染症発症または重症度に関連しているということはなかった。
結論
この高齢者病院で観察された多種の菌株と抗菌薬への耐性に基づくと、これらの結果は、世界の CDI の疫学の全体的な理解の向上に寄与し、高齢者集団を対象としたサーベイランスプログラムの実施に役立つ。

我が国でC. difficileが初めて分離されたのは高齢者専門病院においてである（1979 年）。以来、我が国でも高齢者におけるC. difficileの疫学・分子疫学について知見が集積されてきた。本邦の分子疫学は海外とは異なることが知られているが、本邦の中で、高齢者、非高齢者でどの程度異なるかはまだ情報が少ない。時間的な変化もあることが予想される。高齢者を含む広範囲な分子疫学サーベイランスが継続して行われることが非常に重要である。
