Pre-post evaluation of effects of a titanium dioxide coating on environmental contamination of an intensive care unit: the TITANIC study

背景
欧州の病院または集中治療室（ICU）に入院した患者のそれぞれ 5.7％、19.5％が医療関連感染症（HAI）に罹患しているとされ、抗菌薬耐性が出現している。病原性を有する可能性のある細菌によって病院表面は汚染されるので、HAI 低減に向けて環境の清浄度は重要な側面である。
目的
ICU の環境表面の微生物コロニー形成の低減を目的とした二酸化チタンコーティングの有効性について検討すること。
方法
ICU 室内表面の微生物コロニー形成に二酸化チタンコーティングが及ぼす影響を評価するために、単一施設での前向き比較対照パイロット研究を実施した。介入前と介入後の期間に、表面を接触寒天培地（BBL RODAC 培地）で培養した。表面の細菌コロニー形成に影響する可能性のある因子を記録した。介入の影響は、階層的マルチレベルフレームワーク内の反復測定分析を用い、説明変数で補正して分析した。
結果
介入前と介入後の期間において、ICU 室内のコロニー形成単位（cfu）総数の平均比は 0.86（標準偏差 0.57）であった。至適モデルは以下の説明変数であった：介入（P ＝ 0.065）、週（P ＝ 0.002）、培養表面（P ＜ 0.001）、ICU 室内（P ＝ 0.039）、介入と週の相互作用（P ＝ 0.002）、週と培養表面の相互作用（P ＝ 0.031）。介入前と介入後の期間の 4 週目に、すべての培養プレートの cfu 数に介入が及ぼした影響は－0.47（95％信頼区間－0.24 ～ －0.70）であった。
結論
本研究では、ICU 室内表面の微生物コロニー形成に二酸化チタンコーティングが無効であることを明らかにした。
