Impact of an infectious disease specialist on antifungal use: an interrupted time-series analysis in a tertiary hospital in Tokyo

背景
抗菌薬適正使用支援プログラムは、患者転帰の改善、有害な帰結の件数減少、耐性の予防、および費用効果的な治療の確実な実施を目的とした抗微生物薬使用の最適化にとって、極めて重要であると考えられている。
目的
抗真菌薬に関する抗菌薬適正使用支援プログラムの有効性および限界を評価すること。
方法
2010 年 10 月、感染症を管理するためのバンドルを当院に導入した。2006 年 4 月から 2016年 5 月における抗微生物薬使用密度に関するデータを収集した。抗微生物薬使用密度の傾向を、分割時系列モデルを用いてバンドル実施前、バンドル実施中、および長期追跡調査中という異なる 3 期間について評価した。主要転帰は静注抗真菌薬の抗微生物薬使用密度（1,000 患者日当たりの 1 日規定用量）、副次的転帰は会計年度当たりの抗真菌薬に対する支出とした。
結果
すべての静注抗真菌薬における抗微生物薬使用密度は、2006 年度の 26.1 から 2015 年度には 9.9 に低下した。バンドル実施前の期間における傾向の変化は有意ではなかった（傾き 0.062、95％信頼区間［CI］－0.180 ～ 0.305）のに対し、バンドル実施中の期間には有意な減少が認められた（傾き 0.535、95％CI：－0.907 ～ －0.164）。長期追跡調査の期間中には、傾向の減速が示された（傾き －0.040、95％CI：－0.218 ～ 0.138）。抗真菌薬に対する総支出は、2006 年度の 52,354,411 円から 2015 年度には 14,073,099 円へと 73％減少した。
結論
本バンドルにより、有意に抗真菌薬の使用が減少し、経時的に費用が節減されたが、3 年以内に一定に達したように、その効果は限定的であった。

著者らの述べている抗真菌薬バンドルには、①レジデント、フェロー、担当医に対して毎週 45 分間の感染症診療講習会の開催、②抗菌薬処方前に血液培養検体の 2 セット以上の提出、③喀痰・尿・はじめとする無菌的でない領域からの真菌検出時の抗真菌薬のルーチンの処方禁止、④抗菌薬適正使用支援チーム活動である。一般細菌とは異なる抗真菌薬処方についても、専門家の立場から適正使用の徹底をはかるべきである。
