The potential contribution of 16S ribosomal RNA polymerase chain reaction to antimicrobial stewardship in culture-negative infection

広域スペクトル抗菌薬を用いた経験的療法により、得られた試料で培養陰性が示され、治療の根拠付けが困難になることが多い。本研究では、患者 60 例から得た試料 78 個における 16S rRNA PCR の結果を後向きに検討した。16S rRNA が 28 例（47％）で検出され、5 例（21％）で抗菌薬が de-escalation された。細菌 DNA は 32 例（53％）で検出されず、2 例（8％）で抗菌薬が中止された。脳神経外科患者では陽性結果（53％対34％）および治療の根拠付け（17％対12％）の割合が高かった。特定の患者群において、16s rRNA PCR は抗菌薬療法の対象を絞る上で有用な抗菌薬適正使用支援ツールである。
