グラム陰性桿菌（GNB）が原因の血流感染症の小児における抗菌薬管理は困難な場合がある。本後向きコホート研究では、電子カルテのデータ要素により、GNB が原因の血流感染症に対する経験的抗菌薬療法を最適化する方法を検討した。カテゴリーに基づく GNB 感染症について個々の患者に合わせた感受性パターンの構築、ならびに原因菌と抗菌薬のミスマッチを最小化し、また有効な治療を開始するまでの時間を短縮する機会の特定によって最適化が可能かどうかを調査した。本研究の結果から、抗菌薬療法の開始時、変更時および対象を絞る際に、処方における重要な決定時点に実施可能な潜在的戦略が示唆される。

監訳者コメント：
28 床の小児病院における過去 1 年半に発生した 132 処方の解析である。解析の結果 10件の処方が原因菌と抗菌薬のミスマッチと判断された。自施設の感染症を丁寧に解析することがより最適な経験的抗菌薬療法に反映されることにつながると思われる。
