Incidence and prevention of infective endocarditis and bacteraemia after after transcatheteraortic aortic valve implantation in a French university hospital: a retrospective study

経カテーテル大動脈弁留置術（TAVI）後の感染性心内膜炎は、まれではあるが重度の合併症である。グルノーブルアルプス大学病院でTAVIを受けた患者 326 例中、6 例（1.8％）が感染性心内膜炎を、11 例（3.4％）が菌血症を発症した。いずれの症例も TAVI との関連性は認められなかったが、スクリーニングおよびターゲットを絞った除菌が実施されていたにもかかわらず、1 例が黄色ブドウ球菌（Staphylococcus aureus）によるものであった。この結果は、無作為化試験によって本方針のベネフィットおよび費用対効果を評価する必要性を強調するものである。

監訳者コメント：
TAVI 施行例は本邦でも急速に増加している。TAVI の合併症に関する海外での経験も蓄積されつつあるが、感染性心内膜炎発症率は0 ～ 3.4％との報告がある。本研究では発症までの日数（中央値）は 287 日、最初の 1 年での発症率は 1.4％、発症例全体の死亡率は33％であった。鼻腔の黄色ブドウ球菌スクリーニング、および陽性例に対する除菌施行率はそれぞれ 70％、50％であったが、実際に除菌が完全にできた例は非常に少なく、予防効果は不明であった。将来、追試の施行とともに TAVI 施行例の感染症発症率を低下させる試み・対策が実行に移され、実際の減少につながることに期待したい。
