Validation of healthcare-associated infection surveillance in smaller Australian hospitals

ビクトリア州の中小規模病院（急性期病床 100 床未満）において、黄色ブドウ球菌（Staphylococcus aureus）血流感染症（SAB）、メチシリン耐性黄色ブドウ球菌（meticillin-resistant S. aureus；MRSA）感染症およびバンコマイシン耐性腸球菌（vancomycin-resistant enterococci；VRE）感染症の症例発見を評価するため、検証試験を実施した。全体で、病院 20 施設において感染症 142 例が確認された。SAB イベントに対するルーチンのサーベイランスは、感度 74.4％、特異度 100.0％であった。MRSA 感染症に対しては、感度 47.5％、特異度 90.9％であった。確認されたすべての VRE 感染症は正確に報告されていた。未報告の SAB および MRSA 感染症のうち、それぞれ 80％（N = 16）および 83.9％（N = 26）は市中感染症であった。今後のプログラムの改善には、症例定義、特に市中発症例を含むものが適切に適用されるようにするための対象を絞った教育が挙げられる。

オーストラリアのビクトリア州における医療施設関連感染症サーベイランスは、病院の急性期病床数 100 床を基準に 2 つに分けられている。すべての小規模病院（急性期病床100床未満）には、SAB と MRSA および VRE による感染症の報告を 4 半期毎に実施することが求められている。しかしながら、実施されているサーベイランスが精度よく検出され、報告されているかを検証したところ、市中感染の症例が未報告の大半を占めていた。小規模病院は多くは都市部から離れており、感染対策の専門科や感染症専門家へのアクセスも十分でないことが、結果的に低い感度に反映されている。これらを改善するための教育研修プログラムや症例相談窓口がつくられ、現在これらの活動が進行している。
