集中治療室の患者における血流感染に起因する敗血症:無効な経験的抗菌薬療法が転帰に及ぼす影響

背景
敗血症は、英国において最も多い死因の 1 つである。
目的
集中治療室(ICU)での無効な抗菌薬療法の適用率を明らかにするとともに、無効な抗菌薬療法が院内死亡率、ICU 死亡率、90 日死亡率および入院期間に影響を及ぼすかどうかを明らかにすること。さらなる目的は、無効な抗菌薬療法を受けるリスク因子を特定することであった。
方法
本研究は後向き観察研究であり、2010 年 1 月から 2013 年 12 月にかけて Glasgow Royal Infirmary の ICU で実施された。本研究期間中に計 12,000 の血液培養検体を採取し、そのうち 127 検体が臨床的に重要であるとみなされた。死亡率と独立して関連するリスク因子を特定するため、多変量ロジスティック回帰モデルを使用した。無効な抗菌薬療法を受けるリスク因子を特定するため、単変量解析に続いて多変量解析を実施した。
結果
無効な抗菌薬療法の適用率は 47%(N = 60)であった。多変量解析により、ICU 入室から最初の 24 時間以内に抗菌薬の投与を受けた場合に死亡率が低下することが示された(相対リスク 1.70、95%信頼区間[CI]1.19 ~ 2.44)。また、疾患の重症度(敗血症と敗血症性ショックを鑑別する SIRS 基準の定義に基づく)は、死亡率の上昇と関連することが示された(オッズ比[OR]9.87、95%CI 1.73 ~ 55.5)。しかし、無効な抗菌薬療法は、死亡率の上昇(OR 1.07、95%CI 0.47 ~ 2.41)および入院期間の延長(53.2 対 69.1 日、P = 0.348)につながらなかった。真菌血流感染が無効な抗菌薬療法を受けるリスク因子であった(OR 5.10、95%CI 1.29 ~ 20.14)。
結論
敗血症による死亡率は、複数の因子の影響を受ける。本研究では、無効な抗菌薬療法が敗血症の死亡率に影響を及ぼすことを実証できなかった。
