目的
侵襲性真菌疾患の世界的負担は増加している。カンジダ・アルビカンス（Candida albicans）以外のカンジダ性感染症も出現してきているが、C. albicans は依然として真菌血流感染の主な原因である。診断と管理をめぐる議論のために、我々はこの進化を続ける脅威に効果的に対処できていない。症例に基づく本総説の目的は、現行の議論に対処し、ガイドラインを補完する推奨事項を提供することである。
侵襲性カンジダ症の診断
侵襲性カンジダ症の診断は、診断検査と患者のリスク因子を組み合わせて行う必要がある。直接培養が依然としてゴールドスタンダードではあるが、β-D グルカンおよび C. albicans発芽管抗体の両方が、診断を補助するバイオマーカーとして使用されている。保菌状態と侵襲性疾患との鑑別にはスコアリングシステムを利用できる。
侵襲性カンジダ症の治療
カンジダ血症の第一選択薬としてはエキノキャンディン系が推奨され、臨床的安定が得られ次第、フルコナゾールに de-escalation する。高リスクの患者には経験的療法が強く推奨されるが、現在では、より標的を絞った先制的なアプローチが好まれている。予防的療法を支持するエビデンスは依然として弱いままである。
