Infectious gastroenteritis and the need for strict contact precaution procedures in adults presenting to the emergency department: a Danish register-based study

背景
急性感染性胃腸炎は、接触予防策を実施して感染拡大を防ぐ必要がある。緊急入院の時点では下痢の原因が不明であるため、臨床情報に基づき隔離する患者を判断する必要があるが、それには接触予防策の不適当な実施のリスクを伴う。
目的
デンマークの救急部で胃腸炎が発生する頻度（すなわち、隔離の必要性を評価しなければならない頻度）、および糞便検体の結果に基づき患者に対して引き続き接触予防策を実施しなければならない頻度を検討すること。
方法
本研究は、デンマークの救急部を受診した成人の登録に基づく後向きコホート研究であり、3 つのデータ源（Danish National Patient Register に登録された退院の診断、救急部で採取された糞便検体の微生物学的結果、主訴に基づく入院の原因）を使用した。
結果
66,885 件の緊急入院のうち、患者の 4.3％が胃腸炎の特徴を 1 つ以上有していた。胃腸炎の特徴は、主訴が下痢であった入院（1.6％）、糞便検体の微生物学的検査（2.8％）、および胃腸炎の診断での退院（1.7％）とした。糞便検体の検査を受けた患者の 19％でノロウイルスまたはクロストリジウム・ディフィシル（Clostridium difficile）が検出され、引き続き厳格な接触予防策を必要とした。
結論
救急部患者の 4.3％は、接触予防策の開始を評価する必要がある。糞便検体の検査を受けた患者の 19％は、感染性の高い胃腸炎であり、厳格な接触予防策を要した。隔離する患者を判断する手段を開発するには、さらなる研究が必要である。

監訳者コメント：
救急外来を受診する患者における感染性胃腸炎の割合と接触予防策の必要性に関する疫学調査である。インフルエンザや麻疹など、飛沫予防策、あるいは空気予防策が必要な感染症に比べると、感染性胃腸炎のような接触予防策を要する感染症の外来における感染対策はまだまだ十分とはいえない。本論文はそのような状況に警鐘を鳴らす内容となっている。
