手指消毒におけるエタノールのウイルスに対する有効性★

エタノールは医療機関における擦式手指消毒製剤として世界的に用いられている。エタノールはプロパノールよりも強力かつ広範な殺ウイルス活性を有することが報告されている。本レビューの目的は、エタノールの溶液または市販製剤の殺ウイルス活性スペクトルを記述することである。系統的な検索を実施した。選択した研究は、懸濁試験(49 件)および汚染された手指(17 件)におけるウイルスの感染性低減に関するオリジナルデータを含んだ研究であった。80%エタノールは、対象とされた 21 種類のエンベロープウイルスすべてに対して、30 秒以内で極めて効果が高かった。マウスノロウイルスおよびアデノウイルス 5 型は通常、70% ~ 90%エタノールにより 30 秒で不活化されるのに対し、ポリオウイルス 1 型は多くの場合、95%エタノールを除いて非常に強い耐性を示した(いずれもEN 14476 試験の対象となるウイルス)。80%エタノールは、ポリオウイルス、ネコカリシウイルス(FCV)、ポリオーマウイルス、A 型肝炎ウイルス(HAV)および口蹄疫ウイルスに対して十分に効果的でないようである。しかし、95%エタノールの殺ウイルス活性スペクトルは、臨床的に重要なウイルスの大部分をカバーしている。低濃度のエタノールへの酸の追加は、例えばポリオウイルス、FCV、ポリオーマウイルスおよび口蹄疫ウイルスに対する殺ウイルス活性を大幅に改善するが、HAV などの一部のウイルスはやはり耐性が非常に強い。殺ウイルス作用のある適切な擦式手指消毒製剤を選択する際は、病棟で最も多くみられるウイルスに基づいて、また頻回使用の状況下におけるその製剤の使用者にとって受け入れやすさに基づいて行うべきである。

監訳者コメント
芽胞菌と脂質成分の少ない膜に覆われたウイルスに対して、アルコール系消毒薬の効果は限定的であることがよく知られている。対象となる病原体を絞ることができるときには、適切な手指衛生方法を選択する必要がある。
