アシネトバクター・バウマニー(Acinetobacter baumannii)は、これまで新生児集中治療室(NICU)での保菌および/または感染としばしば関連している。本研究では、NICU におけるカルバペネム耐性 A. baumannii 分離株のクローン性拡散について報告する。計 21 株のカルバペネム耐性 A. baumannii が早産新生児から収集された。ポリミキシン B のみがこれらの分離株に有効であった。カルバペネム耐性 A. baumannii 分離株 19 株がクローン性であった(クラスター C、世界的に拡散している ST1 に属する)。全新生児が末梢アクセスおよびβラクタム系抗菌薬の投与歴を有していた。本研究を実施した病院内のクローン型を根絶するには、厳格な感染制御策の実施が根本的に重要であった。

監訳者コメント:
A. baumannii は日和見感染菌であり、免疫能低下患者に重篤な感染症を引き起こし、致死率も高い。近年、カルバペネム耐性が世界的に問題となっている。特に新生児集中治療領域では超低出生体重児の免疫の未熟さと、人工呼吸器装着、広域のβラクタム薬投与、血管留置カテーテル挿入が、カルバペネム耐性 A. baumannii の施設内感染拡大の危険因子として報告されている。しかしながら、本報告のように、「不適切な感染対策」が、これらの耐性傾向の強い細菌の施設内伝播を助長していることは明らかであり、いかにして接触予防策を含めた感染予防策を現場に個人差のないように普及させるかが大きな課題である。
