この多施設共同後向き研究は、中国南部の新生児集中治療室(NICU)におけるイミペネム耐性緑膿菌(Pseudomonas aeruginosa)の感染の危険因子を検討するためデザインされた。全患者は中国南部の NICU 8 カ所にて緑膿菌感染が確認されており、イミペネム感受性緑膿菌群およびイミペネム耐性緑膿菌群の 2 群に分けられた。データはカイ二乗検定およびロジスティック回帰分析を用いて分析し、計 188 例の診療記録をレビューした。多変量ロジスティック解析では、緑膿菌の分離から 2 週間以内のイミペネムによる治療が、唯一の独立危険因子であった(オッズ比 6.409、95%信頼区間 1.926 ~ 21.333)。

監訳者コメント:
本検討は、中国南部の広東省、湖南省、福建省、海南省、広西チワン族自治区の 22 の NICUに入院した 188 例を対象に行われた。イミペネム検出に関する危険因子で多変量解析後に残ったのは、イミペネムによる治療のみであった。NICU における感染症には交絡因子が多く存在するが、その解析における有意な因子の抽出の難しさを反映しているともいえる。耐性パターンの分析、分子疫学的な情報、感染対策の情報がいずれもないことが本検討の限界である。
