Flocked nylon swabs versus RODAC plates for detection of multidrug-resistant organisms on environmental surfaces in intensive care units

多剤耐性病原体の分離を目的とする 2 つの細菌培養法（ナイロンフロックスワブで採取し、液体培地に漬ける方法とRODACプレート［増幅した微生物の検出とカウント］）を比較するため、1 病院の 4 カ所の集中治療病棟にある、接触予防策を行っている患者の 63 病室における 780 の環境表面を検査した。1 つ以上の培養陽性がみられた場所では、液体培地を用いたスワブのほうがRODAC プレートよりも高頻度に目標の微生物を検出できた（37.5％ vs 26.0％、P = 0.06）。この2 つの培養法による結果は中程度一致していたが（κ = 0.44）、大きく不規則な表面と比較して、小さく平らな表面のほうがより一致していた。

ナイロンフロックスワブ法には、スワブへ微生物が吸着してしまう懸念がある一方、液体培地に漬けることで回収率を高める利点がある。RODAC プレートは、やや盛り上がった培地に直接接触させる方法であるが、でこぼこした面や、物体の縁には接着しにくいという弱点がある。個々の手法で回収率が異なることの他、用いる場所と状況を十分考慮して採取法を選ぶべきであることを示した好例といえよう。
