Tobacco use as a screener for Clostridium difficile infection outcomes

後向きコホート研究を実施して、クロストリジウム・ディフィシル（Clostridium difficile）感染症（CDI）の不良転帰に関する臨床的予測ルールの作成における、自己報告による喫煙の有用性について評価を行った。何らかの喫煙歴を有する患者は、喫煙未経験者と比較して、2 週間以内に感染症が治癒する割合が有意に小さかった。疾患再発、30 日以内の再入院、治療完了前の死亡、および CDI の重症度は、喫煙状態とは関連しなかった。

監訳者コメント：
過去の論文から、喫煙歴が CDI 発症に関連すること、喫煙が肺炎を始めとする複数の感染症のリスク因子として知られていることから、本調査が行われた。喫煙歴を有する患者では、CDI 罹患期間の延長傾向が見られることがわかった。筆者も言及しているが、喫煙歴は入院時の確認事項に含まれているため、情報として入手することはたやすく、CDI 発症時の有益な情報になると思われた。
