フランスの University Hospital of Montpellier において、超多剤耐性に近い北京型結核菌による医療従事者の職業感染例が発生した。初発例は、分離株の遺伝子フィンガープリント法を用いて同定された。本報告は、結核発生率の低い国における超多剤耐性結核菌による医療関連感染のリスク、ならびに結核の管理における分子疫学・遺伝子解析の重要性を強調するものであるが、加えて、多剤耐性／超多剤耐性結核菌感染患者のマネジメントにおいて警戒を強めることを求めている。

監訳者コメント：
イソニアジド、リファンピシンに加え、フルオロキノロンないしアミノグリコシドに耐性を来したものを pre-XDR と定義する。本検討では発症した医療従事者が雇用されていた期間全体について、pre-XDR/XDR 結核患者を抽出し、さらに保存株の分子疫学検査・薬剤耐性遺伝子検査を行って、特定の患者とのリンクを明らかにするとともに、5 薬剤による治療開始までの時間を 38 日まで短くすることができた。感染対策・疫学・検査・治療の全体にわたり合理的かつ迅速にマネジメントを進めることができた成功例である。
