First case of vaccine-strain varicella infection as manifestation of HIV in healthcare worker: a case report and review of the literature

弱毒生水痘ワクチン接種は、非免疫不全集団において安全であり、重篤でない有害事象をまれに伴う。医療従事者 1 名が水痘に感染し、曝露調査が実施された。その結果、曝露した医療従事者のうち 1 名が水痘に対する免疫がなく、曝露後に水痘ワクチンを接種していたことが判明した。その後、曝露した当該医療従事者においてワクチン株による播種性水痘および進行性網膜外層壊死が認められ、診断未確定であったヒト免疫不全ウイルス感染が特定された。本稿では、水痘ワクチン接種の重篤な有害事象を精査する。医療従事者の生ワクチン接種は事前に慎重に検討する必要がある。

水痘ワクチン接種後のワクチン株による感染が HIV 感染診断のきっかけになったとする症例報告である。本報告は本症例以外の水痘ワクチン接種後の有害事象事例がまとめられており、ワクチン接種担当者には一読をお勧めする。
