Consecutive yearly outbreaks of respiratory syncytial virus in a haemato-oncology ward and efficacy of infection control measures

背景
RS ウイルスは、免疫抑制患者で重度の気道感染症を引き起こす。
目的
当血液腫瘍病棟で 2 年連続して発生した RS ウイルスアウトブレイクを報告すること。
方法
呼吸器症状を有する血液腫瘍患者を対象に、口内洗浄食塩水を用いて、PCR 法によりウイルス性呼吸器病原体のスクリーニングを行った。
結果
対象患者で骨髄移植を受けた者はいなかったが、全例が基礎疾患として血液腫瘍を有していた。1 回目のアウトブレイクでは 8 例が罹患し（死亡率 37.5％）、2 回目のアウトブレイクでは 12 例が罹患した（死亡率 8.3％）。両アウトブレイクにおいて広範な感染制御策を実施し、さらなる交差伝播の予防に成功した。
結論
両アウトブレイク、ならびに将来のアウトブレイクの可能性および影響を低減する目的で実施された対応策から重要な教訓が得られた。

呼吸器病原体によるアウトブレイクはインフルエンザのみならず RS ウイルスやヒトメタニューモウイルス、パラインフルエンザウイルスなど様々な病原体が原因となる。本報告では病棟閉鎖や有症状者の隔離、咳エチケットや手指衛生の徹底、患者とスタッフのスクリーニングなど 16 項目もの感染防止策が本文には記載されている。このような報告をもとに、平時から呼吸器病原体によるアウトブレイクのマニュアルを見直しておくと良いだろう。
