Caring for a patient with rabies: implications of the Milwaukee protocol for infection control and public health measures

本稿では、狂犬病と検査上確定診断された一症例を管理した際に行った感染制御策および公衆衛生上の措置、ならびにこれらの対策の実施時に直面した課題について考察した。症例の管理には、集中的かつ集学的な協調を必要とする。Milwaukee プロトコールは、その使用を行うことで、これまでヒト狂犬病を5例生存させえたことから、ヒト狂犬病の指針案の一つとして提唱されている。病院および公衆衛生の臨床家間での合意があれば、選ばれた症例でこのアプローチの今後の発展が促されるであろう。

狂犬病症例における感染制御・公衆衛生上の課題は多い。著者らは、狂犬病の薬物療法・全身管理法としてウィスコンシン大学が提唱している Milwaukee プロトコールと英国保健省の狂犬病感染対策ガイダンスに則り、海外からの輸入例を治療・管理した。残念ながら患者は入院 10 日後に自律神経の機能障害で死亡したが、取られた感染制御、管理体制、検査、公衆衛生的なアプローチについて具体的に述べられており、実用上も有用な報告と言える。
