Association between Pseudomonas aeruginosa positive water samples and healthcare-associated cases: nine-year study at one university hospital

目的
フランスの大学病院において、水サンプルの検査結果と緑膿菌（Pseudomonas aeruginosa）による医療関連感染症例の関連を調べること。
方法
完全ケースおよび代入データセットについて、一般化推定方程式を用いて解析した。空間的単位は建物、時間的単位は四半期とした。
結果
2004 年から 2013 年の間に 2,932 の水サンプルが調べられ、17％が緑膿菌陽性であった。高い緑膿菌症例の発生率は、陽性の水サンプルの割合が高いことと関連した（完全ケース解析では P ＝ 0.056、代入データセットを用いた解析では P ＝ 0.031）。この関連は、血液内科病棟および集中治療室を除外すると消失したが、集中治療室のデータのみの解析では有意であった（P ＜ 0.001）。
結論
本研究から、病院内の水道の蛇口の汚染は、易感染性患者を扱う病棟では、緑膿菌による医療関連感染症例の増加をもたらしうるが、その他の病棟では重要な役割を果たさないことが示唆される。

本論文では、水は病棟の蛇口、シャワー、飲用水から採取された。17％が緑膿菌陽性であったとされているが、患者の検体から採取された緑膿菌との遺伝子学的関連性は調べられていなかった。緑膿菌は環境生息菌であり、水道以外の環境管理がどのようになっているのか、気になるところであった。
