Stenotrophomonas maltophilia in a university hospital of traditional Chinese medicine: molecular epidemiology and antimicrobial resistance

ステノトロホモナス・マルトフィリア（Stenotrophomonas maltophilia）は、高い抗菌薬耐性を示す、新興の重要な日和見病原体である。中国東部の伝統中国医学の大学病院において、 S. maltophilia の抗菌薬感受性、薬剤耐性遺伝子プロファイリング、および分子タイピングの解析を行った。スルファメトキサゾール耐性は多剤耐性の指標であることが分かった。スルファメトキサゾール耐性は、sul および dfrA 遺伝子を含むインテグロン（特にクラス 1）により媒介されていた。クローンの伝播を裏付ける証拠がいくつか見つかったことから、病院内で抗菌薬耐性株の交差感染が生じたことが示唆された。このことは、病院内での効果的な制御・予防策の必要性を強調している。

監訳者コメント：
院内で分離される S. maltophilia は遺伝的に多様であることが判明しているが、本検討は薬剤耐性遺伝子に関わるインテグロン構造の有無とタイプまで解析し、交差感染の可能性を論じた点が評価できる。耐性出現と伝播、両方を一括して防止できる方策を探るうえで、有用な情報であるといえる。
