Use of parenteral antimicrobials in very small hospitals in inner Brazil: patterns, determinants, and opportunities for interventions in developing countries

発展途上国における医療の大部分は小規模の病院で行われている。これらの状況下での抗菌薬使用についてはほとんどわかっていない。本研究では、ブラジル中心部にある 50 床以下の病院 48 施設における非経口抗菌薬の 60 日間の使用を調査した。全体での使用量は 1 日規定用量（DDD）で入院 100 件あたり 242.0 DDD であり、広域スペクトルの抗菌薬が 26.8％を占めた。抗菌薬処方に対する病院内でのガイドライン、教育手段、制限方針、ならびに感染管理および微生物学的リソース（レファレンス・ラボラトリー）の存在が、使用量の減少と有意に関連していた。これらの結果は、発展途上国における抗菌薬過剰使用の防止を目的とした介入の可能性を示すものといえる。

監訳者コメント：
薬剤耐性アクションプランを推進していくにあたり、小規模病院での現状把握と対策立案、そして実行は、非常に重要であるとともに、かなりのウェイトを占めるものである。ブラジルで行われた本研究で明らかになった、使用量の減少と相関していたファクターは、本邦の小規模病院での課題とも共通しているものではなかろうか。本邦での課題の克服に参考となる論文の一つといえよう。
