健常新生児室における髄膜炎を伴うエリザベトキンギア・メニンゴセプティカ(Elizabethkingia meningoseptica)による敗血症のアウトブレイク★★

2012 年 3 月から 5 月に、地域産科病院で出生した 3 例の新生児が、生後 10 日未満で髄膜炎を伴うエリザベトキンギア・メニンゴセプティカ(Elizabethkingia meningoseptica)(以前のChryseobacterium meningosepticum)敗血症を発症したが、3 週間から 6 週間のシプロフロキサシン+バンコマイシンまたはピペラシリン・タゾバクタムの静脈内投与による治療が成功した。新生児室および分娩室から採取した 24 の環境サンプルのうち 4 サンプル(16.6%)で、この細菌が検出された。臨床分離株のすべて、ならびにおしゃぶりおよびおしゃぶりカバーの保管箱からの 2 分離株は、遺伝学的に同一であった。保管箱をステンレス製の箱に替え、定期的なオートクレーブ滅菌処理を行った結果、新たな E. meningoseptica 感染の症例は生じなかった。

監訳者コメント:
エリザベトキンギア・メニンゴセプティカは、グラム陰性桿菌で土壌や水などの自然界に生息する細菌で、本菌による院内感染は 1,000 入院患者あたり 0.007 ~ 0.039 とまれであるが、医療処置の多い NICU/ICU で発生しやすく、特に新生児の髄膜炎が報告されている。耐性傾向が強く、抗菌薬の選択が難しく、本菌による重症感染症では死亡率 50%ととも報告されている。本例は一般の新生児室で、医療処置をされていない新生児に発生した集団発生であり、環境汚染(おしゃぶり)から新生児の髄膜炎が発生したと推定されている。過去にこの施設では同一児には同じおしゃぶりを消毒して再使用していたが、この発生を契機に、英国の家庭および病院における母乳作成等に関するガイドライン(Journal of Hospital Infection 92 (2016) 213-221)にしたがって、おしゃぶりは 24 時間毎に交換し、決して消毒して再使用しないとした。
