院内感染リスクの重要な決定因子の一つは滞在期間である。院内感染リスクにさらされている期間の分布は、高度にゆがみ、退院か死亡かによって異なってくる。本研究では、多数の短期滞在患者および少数の長期滞在患者を考慮したランドマーク競合リスク予測モデルを適用した。

監訳者コメント:
医療関連感染の獲得および発症リスクは、一般的に入院期間が長くなるほど高くなるが、一方で入院期間が長期に及ぶ例では、基礎疾患の重症度やデバイスの装着率も高くなる。したがってそれらの影響を排除しないと、医療関連感染のリスクと入院期間との関連は正しく解析できないことになる。ランドマーク競合リスク予測モデルを用いて ICU 入室患者を解析した本検討では(退室のピークは入院 5 日目)、院内感染のハザード(累積でなく、ある時点でのリスク)は入院 20 日目で最も高いことが判明した。
