Legionellosis acquired through a dental unit: a case study

2012 年に、スウェーデン・ウプサラの病院で免疫不全の高齢男性患者がレジオネラ症で死亡した。この患者は、潜伏期間中に当該病院の歯科病棟を受診していた。口すすぎ用コップ給水装置から 2,000 コロニー形成単位/L の濃度でレジオネラ（Legionella）属菌が分離された。患者および歯科病棟からの分離株は、レジオネラ・ニューモフィラ（Legionella pneumophila）血清群 1、サブグループ Knoxville および ST9 であった。パルスフィールド・ゲル電気泳動および全ゲノムシークエンシングにより、分離株は共通の原因に由来することが強く示唆された。本報告は、歯科病棟から感染したレジオネラ症の数少ないケースのうちの 1 つである。

監訳者コメント：
気管支肺胞洗浄液からの分離株と、口すすぎ用給水装置からの分離株とが、ゲノム全体でみても極めて相同性が高かったことから、歯科での使用水を介したレジオネラの伝播について、より強い証拠を得ることができた事例である。これまでの報告では解析結果に疑義があり、ここまで精密な情報が得られたのは初めてとはいえ、今後、同様の事例の集積とさらなる解析が進むことに期待したい。
